-Joint photograph
-Request letter for the change of name
-Original Marriage Certificate and a copy thereof.
-Copy of passport of the spouse.
**If there is change of name due to other reasons;
-Advertisement of the change of name in any leading newspaper at the permanent address in India and at the present address in Japan (original copy of the newspaper should be shown at the counter)
**For deletion of name of spouse from the passport
-Original and copy of Divorce Decree certified by the Ministry of External Affairs, India if the marriage was solemnized in India or;
-Original and copy of Divorce Decree translated in English by professional translator and certified by the Ministry of Foreign Affairs, Japan if the marriage was solemnized in Japan.
**For inclusion of Spouse’s name on passport
-Original Marriage Certificate certified by the Ministry of External Affairs, India if the marriage is solemnized in India and a copy thereof or;
-Original Marriage Certificate translated in English by professional translator and certified by the Ministry of Foreign Affairs, Japan if the marriage is solemnized in Japan and a copy thereof.
- Completed application form with thumb impression (as prescribed in the passport form) of the child.
- Child’s Residence Card and a copy thereof. If Resident card is not yet ready, please submit a copy of the receipt of application from the Immigration office.
-Duly filled in and signed Miscellaneous Services Form
- Original passport (to be shown at the counter) and a copy thereof. - Alien Registration card or Residence Card(to be shown at the counter) and a copy thereof.
-One passport size photograph (5cm x 5cm)
-Application to the Consular Officer on a plain sheet of paper explaining the purpose for which the service is required.
- Fill Emergency Certificate Form along with passport (even if expired) and a copy thereof. - If original passport is not available, supporting evidence of Indian nationality like passport copy / driving license with a copy / alien registration card with a copy etc. should be submitted. - Documents issued by the Immigration Authority, if any. - Five (5) recent passport-size photographs with light background (5cm x 5 cm) and specification in https://www.indembassy-tokyo.gov.in/downloads/PhotoSpecs.pdf
-Request letter explaining the circumstances warranting issuance of Emergency Certificate.

**For change in name, change of address, inclusion of spouse’s name, deletion of spouse name or any change of information on passport, the applicant has to submit fresh passport application along with the required supporting documents.
The above mentioned documents are general mandatory requirements. The applicants may be requested to submit some more documents after scrutiny of the application at the counter, on case to case basis.
